Call for an appointment. Hours at Outpatient Care Broadview Heights Center are: M-Th: 7:00am-7:00pm; F: 7:00am-5:00pm; Sa: 8:00am-12noon. Hours at the main hospital are: M-F: 8:00am-4:30pm. A referral is required in writing by a physician or case manager for the initial visit and every time there is a new diagnosis. Referrals may be faxed

Those who have no health insurance must have household income at or below 400% of federal poverty guidelines to apply for financial help. Those living further than 150 miles from the main site may have other requirements.
