*Fairfield County Resident or Hocking County for Logan location only *Income Based: $69,000 or less *OR* *Older adults aged 60 or older

*Valid photo ID (you and your spouse) *Social Security cards or statements for everyone listed on the return *All wage/income documents *Interest and dividend statements, if applicable *Self-employment information: income, expenses, mileage, & 1099-NEC/k forms *Copy of last year's return, if available *Banking routing & account number for direct deposit *Form 1095-A (Health Insurance Market Place Statement) *IRS Identity theft pin if you receive one
