Provides a coordination of support services which enables individuals to access services that may be needed to ensure a successful pregnancy. Services also available for the newborn child. Services include referrals to WIC, individual counseling and education, coordination of medical appointments before and after the child is born, group education; Lamaze classes, parenting and child care classes, nutrition intervention, transportation to Medicaid-reimbursed services, prenatal risk assessment, and prenatal care.
