Eligibility
Assistance with only one ID per person in 12 months, and two birth certificates per family in a life-time. Those seeking assistance will be asked to provide one piece of "identification". This could be a Social Security card, a piece of mail, an expired ID, birth certificate, etc. Anything that may have their name on it.
Required documents
Social security card, health insurance card(s), birth certification, student ID or similar form of identification should be brought.

Application process
All are welcome on Wed. between 10:00 am -12:30 pm appointments are no longer needed.

Offers financial assistance for individuals seeking state-issued photo identification cards or birth certificates.
  • Limit of 2 birth certificates per family, given one time only. If seeking birth certificate for children, must provide proof of relationship to said children.
  • Limit of 1 state ID card per year.
*Help with obtaining birth certificates from other states offered, but requirements from other states may hinder the ability to obtain said birth certificate.
