Eligibility
Covers children age birth up to age 19 in families earning up to 206% of Federal Poverty Level, and pregnant women earning up to 200% of Federal Poverty Level. Must reside in Lucas County and be a citizen or be in a qualified immigration status per Medicaid requirements. Families earning up to 156% of Federal Poverty Level are permitted to have other insurance, and Healthy Start will always be the payer of last resort; children in families earning up to 206% of Federal Poverty Level may not have any other applicable insurance. (NOTE: Federal Poverty Guidelines are set at monthly gross income limits)
Required documents
Agency can look up most supporting documents through an electronic records search - the client will be notified if they need to supply any extra supporting documents. If necessary, the client may need to provide: Proof of income Applicants who are U.S. citizens and not receiving Medicare or SSI will have to show proof of citizenship (primary documents for this purpose are a valid or expired passport, certificate of naturalization, or certificate of U.S. citizenship) Those who do not have one of these will need TWO documents: a certified birth certificate, hospital birth record, or religious birth record made within 3 months of birth, PLUS an identity document, which in most cases is a government issued ID such as state ID or driver's license. They may also need documentation of name change for any reason. If client cannot afford the necessary documents, or documents are unavailable, agency assist the client to obtain them or find other options. Services will NOT be denied or terminated because of this requirement, as long as client makes an effort to comply.

Fees
No fee to apply. Once receiving coverage, please find medical providers within the Managed Care Plans (MCP) network. Individual MCPs may charge co-payments for some services.

Provides health insurance through a Managed Care Plan (MCP) for children and pregnant women in low-income families. Services for children include hospital or acute illness care, well-child checkups, limited dental care, eyeglasses, prescribed medication, and immunizations. For pregnant mothers, will cover doctor visits before and for 12 months after delivery, prescriptions, and the delivery itself. Healthy Start includes Medicaid and Children’s Health Insurance Program (CHIP). The Medicaid program is available to: • Insured or uninsured children (up to age 19) in families with income up to 156% of the federal poverty level. • Pregnant women in families with income up to 200% of the federal poverty level. The CHIP program is available to: • Uninsured children (up to age 19) in families with income up to 206% of the federal poverty level. Healthy Families is a Medicaid program available to: • Families with income up to 90% of the federal poverty level and a child younger than age 19
