Eligibility

Be diagnosed with end-stage renal disease or have recently received a kidney transplant.
Live or dialyze within the 20-county service area of the Kidney Foundation of Northwest Ohio.(Allen, Auglaize, Crawford, Defiance, Fulton, Hancock, Hardin, Henry, Logan, Lucas, Mercer, Ottawa, Paulding, Putnam, Sandusky, Seneca, Van Wert, Williams, Wood, Wyandot)
Be under the care of a social worker who can apply for assistance using the Kidney Foundation’s online application
Be approved by a member of the KFNWO staff

A social worker should submit a bill or invoice via the service entity

Provides emergency assistance of up to $150 per calendar year for rent or utilities. Emergency Assistance is intended to supplement the cost of a particular need and is not necessarily intended to cover the full cost. Emergency assistance will not be dispensed directly to a patient as a cash payment.
