Other eligibility may apply, but generally: Adults 18 and older who are terminally ill, but do not yet qualify for Medicaid because they have a life expectancy longer than 6 months; Youth under 18 who are terminally ill (generally have a life expectancy of 1 year or less) or have a life-limiting diagnosis and are in a state of decline. Serves Ashland, Ashtabula, Cuyahoga, Erie, Geauga, Huron, Knox, Lake, Lorain, Medina, Ottawa, Portage, Richland, Sandusky, Seneca and Summit counties, with outreach to Morrow, Stark and Wayne counties.

Phone 24 hours a day, 7 days a week for enrollment information and to schedule an in home assessment. An assessment will be done to see if the person is appropriate for this program.

Fixed fee but no one is denied service because of an inability to pay. Third party payment accepted: Medicaid; Medicare; Private insurance.

Description

Provides palliative pain and symptom management, care, and emotional support for persons with a life-limiting illness (life-threatening or terminally ill) and their families. Provides supportive services such as bereavement counseling, social work, art and music therapy, and spiritual care. Respite care is provided for client's families.
