Eligibility
Special needs children with qualifying condition up to age 21. All forms and information can be found on the Humanitarian Foundation website https://hfgrotto.org/.
Specified Medical Condition/Diagnosis covered by this program are
Cerebral Palsy, Muscular Dystrophy and related neuromuscular diseases, Intellectually Delayed, Dental Treatment for Organ Transplant recipients, and Cleft Lip and Palate (through a Shriners Hospital Only).
Intellectually Delayed covers profound to two (2) years OVERALL intellectually delayed in their learning abilities and cognitive level. A signed letter from the physician or licensed school psychologist stating the delay must accompany the application.
