HOME SUPPORT SERVICES | CUYAHOGA COUNTY DIVISION OF SENIOR AND ADULT SERVICES

Provides skilled home nursing, home health aides (to assist with grooming, ambulation, personal care, and bathing), in-home physical, occupational, or speech therapy, and in-home respite care for eligible adults in need of these services. Also provides homemaker assistance (meal preparation, laundry and general household management including light housekeeping.) Services also include personal care and homemaking for adults with HIV/AIDS.

Application process

Phone for appointment or information. Basic information will be taken, and a visit from a nurse will be scheduled to assess the case.

Fee

Medicare accepted for skilled nursing. Fixed fee for those without Medicare; however, there is a sliding scale for personal pay services. Services for those with HIV/AIDS are paid for through Ryan White Part A funds; partial subsidy is available for those with multiple sclerosis through the MS Society.

Eligibility

Open to residents of Cuyahoga County who are age 60 or older, or who are ages 18-59 with a disability. Skilled nursing must be prescribed by a physician, other services available without prescription.
