Eligibility

Adults age 19-64 who are not currently receiving Medicare. Must live in Lucas County and be a U.S. citizen or in a qualified immigration status per the United States Citizenship and Immigration Services (USCIS). Financial eligibility is based solely on income, and is set at 133% Federal Poverty Level (FPL).

Required documents

Agency can look up most supporting documents through an electronic records search - the client will be notified if we require extra supporting documents to determine eligibility for the program. If necessary, one or more of the following may be requested: Valid Social Security Number (SSN), Ohio Residency if in question, Proof of U.S Citizenship or qualified non-citizenship status, proof of income, proof of Primary Insurance, proof of any student loan interest payments being made (allowable deduction from income). Applicants who are U.S. citizens will have to show proof of citizenship. Primary documents for this purpose are a valid or unexpired passport, certificate of naturalization, or certificate of US citizenship those who do not have one of these will need TWO documents: a certified birth certificate, hospital birth record, or religious birth record made within 3 months of birth, PLUS an identity document, which in most cases is a government issued ID such as state ID or driver's license. They may also need documentation of name change if applicable. If client cannot provide the necessary documents, agency worker will assist the client, upon request. Services will NOT be denied or terminated because of this requirement, as long as client makes an effort to comply.

Fees

No fee to apply. Once receiving coverage, please find medical providers within the Managed Care Plans (MCP) network. Individual MCPs may charge co-payments for some services.

Description

Provides public insurance to cover medical services that may include doctor visits, some prescription drugs, hospital care, home care, long-term care, medical equipment, transportation, mental health, vision, dental, substance abuse services and other medical services. Once approved for Medicaid, clients will be auto-enrolled into one of the seven Managed Care Plan options (MCP). Medicaid recipients are required to renew their coverage annually and will receive Medicaid Renewal packets before their 12-month Medicaid Certification period expires.
