Eligibility

Most counseling available to individuals, age 18 and older, diagnosed with a mental health disorder. Also serves individuals dealing with trauma or emotional distress, wanting to improve communication, coping skills, increase self-esteem, and promote overall wellbeing. Also serves families.

Required documents

State ID, proof of income, proof of residency, proof of insurance, birth certificate, Social Security number.

Phone 24 hours a day, 7 days a week for information or M-F: 9:00am-6:00pm to make an appointment.

Fees

Services are billed to Medicaid and Medicare. All MCOs and managed care plans are accepted. Additional funding is available for those without insurance.

Provides comprehensive biopsychosocial (diagnostic) assessments, counseling services, and community-based case management services. Some of the services include grief, trauma, and bereavement counseling, substance use disorder counseling, and family counseling.
