Provides non-emergency medical transportation for Medicaid eligible recipients under the Non-Emergency Transportation (NET) plan. Provides transportation to out-of-county medical providers (for example, Cleveland, Akron, or Columbus) and local medical providers. Recipients must be eligible for Medicaid to use the service. 10 day notice is required.

Application process

Call the Non-Emergency Transportation Request Line: 419-774-5400 and select option 3. Requests for transportation services must be received at least two weeks prior to the appointment date. Leave name, Social Security number, phone number, appointment date/time, appointment address and physician's name spelled out.
