Eligibility
Individuals in Lake County with severe mental illness who are homeless and do not require 24-hour supervision.

Monday - Thursday: 8:30 am to 4:30 pm.
Friday: 8:30 am to 4:00 pm.

Application process
Referrals primarily through the Lake County ADAMHS Board network of providers, e.g. Beacon Health, Signature Health.
Fees
Participants pay 30% of their adjusted gross income or 10% of total income (whichever is higher) towards rent and utilities.
