Program provides prescription sports goggles to vision-impaired youth from low-income families. Goggles can be used in a variety of sports, including basketball, baseball, soccer, racket sports, girls lacrosse and field hockey.

Application process
Phone to receive paper application via mail, or apply online (playhardgear.com). May be contacted by staff to confirm eligibility. Eligible clients will be mailed a voucher, which can be used to order free goggles through a participating eye doctor (list of doctors will be included in mailing). Client must provide doctor with copy of child's prescription at time of order.

Eligibility
Youth age 5 to 17, with impaired vision (can be common refractive error or more serious impairment), whose family's annual income is at or below 300% of the Federal Poverty Guideline (FPL).
