18 months and over. Pregnant women must provide written consent from their primary care physicians or OB/GYNs prior to receiving treatment at FastCare.
General required documents include Photo ID and proof of insurance.
Mon-Fri 8:30 am-8:30 pm. Saturday 9 am-6:30 pm. Sunday 10 am-4:30 pm. Open all holidays with the exception of Christmas and Easter. Call to confirm hours.

Visits start at $59. Accepts most insurance providers, CareSource, Molina, Medicare, Medicaid and private pay. $39 for physicals. Physicals cannot be billed to an insurance carrier.
