Recovery supportive services available for those with substance use and addiction issues. Also assists with Medicaid enrollment. Provides HIV/STI testing and other prevention services, including PrEP. Provides HIV care and case management and linkage to care.

Application process

Telephone for appointment. Walk-ins and same-day appointments accepted. Primary care medical appointments and vision appointments are available same-day. Telehealth visits are available for follow-ups. Dental services are available Monday-Thursday for hygiene and acute needs. Walk-ins encouraged. Accepts most insurance including Medicaid and Medicare. Sliding fee scale for uninsured residents.

Eligibility

Low income individuals with a focus on clients who experience housing instability, homelessness and other issues making care difficult to access.
