Those 18 and older who have been diagnosed as terminally ill and are under the care of a hospice. Must have limited financial resources and no available caregivers. Must be free of tuberculosis.
Documentation of diagnosis by physician
Signed "do not resuscitate" order
will be advised of other needed documents.
24 hours a day, 7 days a week.
Referral must be made by a hospice social worker. Phone for information M-F: 8:00am-5:00pm.

Provides a residential care facility for terminally ill adults who are in the final stages of life. Trained staff and volunteers provide spiritual, emotional and physical support along with the assistance of the individual's hospice team.
