Eligibility

Eighteen years of age or older and residing in Cuyahoga, Geauga, Lake, Lorain, Medina, or Summit counties. Must have a dual diagnosis of mental illness and chemical dependency. Accepts Medicaid-eligible pregnant women.

Required documents

Proof of income if no insurance. Proof of insurance if applicable. Medicaid consumers residing outside Cuyahoga County may have to show additional proof to obtain coverage.

Hours

Depending on phase of treatment, meetings will be during the day, during agency hours, M-F: 9:00am-5:30pm. Aftercare program takes place during evening, weekday hours.

Application process

Phone for intake. May have to leave a message that will be returned. De-toxification required prior to beginning program if screening reveals a need.

Fees

Accepts private payments, limited third party payment (Medicare and Medicaid). Accepts Medicaid-eligible pregnant women before they are receiving Medicaid. Will work with individual pregnant women who become clients to submit Medicaid applications.

Description

Offers team approach to a comprehensive, outpatient substance abuse treatment program for those with both a mental health and substance abuse diagnosis. Program includes mental health and substance abuse diagnoses, case management, medication monitoring, individual and group therapy, treatment, and aftercare.
