Eligibility

Cuyahoga County residents who have a severe mental illness such as schizophrenia, major depression, or bipolar disorder. East Health and Wellness Center serves age 18 and older - no age restrictions for all other sites. Client should be enrolled in Medicaid or income eligible for Medicaid (138% of the Federal Poverty Limit). If the client is not yet enrolled for Medicaid, the agency will screen for eligibility and help them to get enrolled.

M-F: 8:00am-5:00pm

Application process

Phone for screening M-F: 9:00am-4:00pm. After screening, eligible clients will be instructed to walk in to an appropriate site for an initial psychiatric assessment M-F: 9:00am-3:00pm, in some cases, this may be the same day. ALL INDIVIDUALS MUST HAVE A CASE WORKER. If you do not have a case worker, you can call for your mental health screen and a case worker can be assigned to you. Mental health assessment must be completed, once completed, an appointment for an assessment will be made. Once the assessment is complete, if appropriate the client will be enrolled for services. Following assessment, client should be scheduled for a psychiatric counseling appointment within 3 to 10 days. Entire process from first phone call to first appointment should take no longer than two weeks.

Fees

Accepts payment through Medicaid, however, client does not have to already be enrolled in Medicaid for initial intake.

Description

Offers case management for individuals with severe mental illness to help them succeed at school, work and in the community. Participants also receive additional services such as referrals, support groups, medication monitoring services, prescription assistance, daily living skills support and primary health care services. Psychiatric counseling and medically necessary mental health services are provided. A psychosocial assessment is included as part of the process to determine treatment needs. Services may be short or long term. Will handle alcohol/chemical dependency only as a secondary issue.
