Phone for an appointment M-F: 8:15am-4:30pm for any services, including free screenings. Referral from physician required for services other than hearing screenings.
Fee
Third-party payment accepted, including Medicaid and Medicare. Those who have no health insurance must have household income at or below 400% of federal poverty guidelines to apply for financial help. Financial Counselors will screen uninsured for Marketplace insurance, Medicaid, or hospital charity eligibility.
Eligibility
Ages 18 and older for free screenings, School age (5 and up) for hearing aids. Serves all ages for other services.
