Description
Provides an integrated program of home care services designed to address the specific home care needs of those under the age of 60 needing nursing and/or daily living services. Two types of benefits packages are available: Home Health Services, and Waiver Benefits. Home Health Services provide nursing, daily living, and skilled therapy. Waiver Benefits provide the same services as Core Services but may also include: home delivered meals, emergency response systems, home modifications, adult day health services, out-of-home respite care, social work, and care coordination. (Upon turning 60, waiver recipients are automatically transferred onto the Transition II [Carve-out] Waiver, which is only available to these individuals, and cannot be applied for independently).
