Eligibility
Serves residents of Portage County that meet all the following criteria:
- Must live in eligible delivery area (lives within 10 miles of Center of Hope located at 1081 West Main Street in Ravenna, 44266).
- Household income is at or below 200% of the Federal Poverty Level.
- Is experiencing health concerns AND transportation barriers.
