Application process

Application, participation agreement, fee, and Disability Verification Letter must be mailed to Columbus office with ATTN to Eric Rathburn. Contact for more information.

Fees

$60.00 if delivered by mail. $30.00 if in-person pick up at office. Credit and debit not accepted, payment must be made in cash or check.
