Eligibility
- Resident of Sandusky County
- Referral from Sandusky County Job Store demonstrating registration with the Sandusky County Job Store or be seeking subsidized housing and have proof of appliciation
- Must have been born in Ohio to receive assistance with a notarized Birth Certificate.
-May only receive assistance for Identification one time
Required documents
- Social Security Card or Social Security print-out
- Proof of residency
- Birth Certificate (unless requesting assistance for Birth Certificate)
- Completed application

Application process
Applications are available through the Sandusky County Job Store and area subsidized housing offices. Applicant is then responsible for setting up appointment at the Liberty Center to provide documentation and pick up voucher for birth certificates and/or ID.

$2 for each Identification request
