Client must live in Cuyahoga County. Serves those whose developmental disability occurred before the age of 22.
Birth certificate Social Security number documentation that verifies the developmental disability.
Office hours are M-F: 8:00am-4:30pm, however someone is available to take calls 24 hrs a day, 7 days a week in emergencies.

New clients should phone the Eligibility line to initiate service, to find their case worker (if they are having trouble finding them)or for more information about Board services. Those who are already being served by this program and know who their Support Administrator (also called case worker) is (and what office they are at) can call that office directly.

Description

Helps eligible individuals identify and coordinate needed services. Develops and monitors the Individual Service Plan (if the person has one). Provides information and linkages to community-based resources.
