Eligibility

Must be a dialysis or transplant patient and live within the service area. Income must be at or below 200% of the federal poverty level for ongoing assistance. $150 of emergency assistance is available to any dialysis or transplant patient with no income qualification needed.

Required documents

Referral from the patient's social worker at the dialysis or transplant center where treatment is being provided.
