Interested applicants can make an appointment to complete the intake/interview process by contacting the M.O.M.S. Care Coordinator. Interviews are conducted during the initial enrollment period, 3 months after enrollment, 6 months after enrollment, and at the end of the program. Interviews are kept completely confidential.

Description
A program to improve the health of pregnant women who have been diagnosed with an opiate or amphetamine use disorder and their newborn children. Care coordination and wrap-around services include: medication-assisted treatment options, dual-diagnosis counseling services, psychiatric services, childcare services, transportation to and from medical appointments, recovery support services, housing assistance, OB/GYN and pediatric referrals, and supported employment services. Eligible applicants can enroll at the onset of pregnancy and stay in the program for up to one year after the delivery of their infants.

If you are one of those individuals they will contact you to complete a phone health screening prior to your appointment. If you are an established client, your provider will be reaching out to you. Their pharmacy partner, Genoa Healthcare, will remain open but will be focused exclusively on filling your prescriptions by mail.
