Application process

Phone M-F: 9:00am-5:00pm to request service. Acceptance of new patients may be on a case-by-case basis depending on their situation, location, etc. If individual who needs service already has a primary care physician, must get a written referral from physician before getting service.

Fee

Third-party payment accepted, including Medicaid and Medicare. Those who have no health insurance must have household income at or below 400% of federal poverty guidelines to apply for financial help. Financial Counselors (FC) will screen uninsured for Marketplace insurance, Medicaid, or hospital charity eligibility. Patients who qualify for charity might not always have a 100% of bill covered. Those who have an outstanding bill may be able to receive a payment plan on a case-by-case basis. For patients with insurance: program does not cover out of pocket expenses such as co-pays, co-insurance, and/or deductibles.

Eligibility

Serves Cuyahoga and Lake Counties and portions of Geauga, Lorain, Medina, Portage and Summit counties. Eligible individuals are homebound, generally aged 65 and older, although younger individuals will be served pending approval by the board of the agency.
