A referral must be made by the Board of Developmental Disabilities in which the individual resides (Cuyahoga, Lake, or Stark only). Once approved, the application can be filled out and completed. Once the application is submitted and approved, a member of NCCH will meet with the individual to determine their needs and finds an appropriate home for them. The individual will be placed in an existing home, however, if necessary, NCCH will then purchase a home, make any necessary renovations, and manage the property. Generally, four people with developmental disabilities live in one home. Call for further information M-F: 8:00am-5:00pm. In Summit County referrals are done through ADAMHS Board.
Fees vary depending on location of home and direct service provider organization selected by the individual to provide supportive services in the home. Fees are often paid through a funding source such as the County Board of Developmental Disabilities or Section 8.

Description
Develops and maintains homes for people with developmental disabilities (MR/DD). Assists with placing individuals with developmental disabilities into transitional housing to prepare them for independant living. North Coast Community Homes may purchase homes, makes any necessary renovations, and manages the properties of those who qualify for their assistance.
