Application process

Return completed AOC-700A form to County Attorney's office. Forms are available in person or online. Note: At the time the form is returned, appointments must already be scheduled for the person for which treatment is being sought to be evaluated by two Qualified Health Professionals, one of which must be a licensed physician.

Fee

Varies. The person seeking the involuntary treatment is obligated to pay all costs incurred in the process, as well as all cost of treatment and must sign a guaranty of payment before the case begins.

Eligibility

A spouse, relative, friend, or guardian of the person may file a petition/request with the County Attorney.
