Description
Once an appointment is scheduled, client will be required to complete a financial intake where payer sources are identified and other information is collected, including completion of health & outcome forms, then will meet with a licensed mental health staff member to complete a diagnostic assessment. This might carry over to the second appointment depending on the circumstances. A counselor/therapist will then be assigned to the client’s case.
