Ohio residents of any age. Ohio residency is established when individual adults (18 and over) who have newly moved to the state take a job, sign a lease, buy a home, register to vote, or enroll children in school.

Must present documents to prove all the following five elements: (1) Full legal name: current first (given) name, middle name, and last name (surname) without the use of initials or nicknames (Birth Certificate, Passport or Passport Card can be used). If name on documents does not match the person's name, must present the document (original, not a copy) that states the name has been changed (2) Date of birth (Birth Certificate, Passport or Passport Card can be used). (3) Social Security Number (SSN) (SS Card, W-2 or tax form, or 1099 Income Statement can be used), if ever assigned (4) U.S. citizenship OR U.S. legal presence (Birth Certificate, Passport or Passport Card can be used) and (5) Residential street address in Ohio (Official Business or Official Government Document with home address can be used). In the case of a dependent child, the agency may accept a certified statement of residency from a child's parent or guardian, signed in the presence of an agency official. Satisfactory proof of identity, Ohio residency, and relationship of the parent or guardian to the child applicant is also required. Non-US citizens are required to provide proof of legal presence. Proof of residency will be required to renew an identification card if there is a change of address not reflected on the expiring card. NOTE: for Compliant ID Card must present TWO documents from different sources proving Ohio Street Address that are listed on the Acceptable Document List are required for proof of Ohio residency.
