Eligibility
- Serves students new to the Tallmadge City Schools district.
- Children must be age five (5) by the first day of the school year in order to attend Kindergarten during the academic year.
Required documents

- Photo ID of the parent/guardian.
- Student's Birth Certificate.
- Student's Immunization Records.
- If child has an IEP (Individualized Education Program), a current copy along with the current ETR (Evaluation Team Reports).
- Proof of Custody (if applicable): Certified copy of court order or court appointed guardianship.
- If student is in the custody of Children's Services: Must present an Affidavit of Custody and Proof of Educational Responsibility.

If you do not have access to a computer at home or need assistance with the online registration, call the office and speak with the Registrar at extension 8211. Once the forms are completed, call the Registrar for information on completing the registration process.
