Eligibility
Serves adults age 60 years or over who live in Belmont, Carroll, Coshocton, Guernsey, Harrison, Holmes, Jefferson, Muskingum, or Tuscarawas county.
Required documents
Will be requested as needed.
Hours
Monday - Friday: 8:00am to 4:30pm. Actual in-home services for respite and homemaking are available anytime.

Application process
Call the office Monday through Friday from 8:00am to 4:30pm and ask for a screener. They will ask questions and provide information regarding required documentation and the process. A face-to-face assessment will be scheduled with a Registered Nurse or Licensed Social Worker.
Fees
Case management is free; sliding scale for other services.

Provides geriatric case management services that include consultation, evaluation, and planning a course of action for older adults. Offers options and makes recommendations for care. Works with the client and the client's family members, physicians and other professionals such as attorneys. Service includes explaining what is available, discussing eligibility requirements, financial resources required, determining needs and wishes of clients, and helping to arrange services. The family and the client are given a community care plan for their use.
