PHYSICALS/IMMUNIZATIONS/PEDIATRICS | CLEVELAND CLINIC FOUNDATION

Provides preventive medical care services such as physicals, childhood and adult immunizations, checkups to follow the development of babies, screening for lead poisoning through blood tests and the services of pediatricians. Provides vaccination against shingles at some sites. Also provides immunizations against diseases that may be encountered by those traveling to countries outside the US.

Hours to make appointments for all services are generally M-F: 8:00am-5:00pm. There may be a waiting list for shingles vaccinations at some locations.

Fee

Third-party payment accepted, including Medicaid and Medicare. Those who have no health insurance must have household income at or below 400% of federal poverty guidelines to apply for financial help. Financial Counselors will screen uninsured for Marketplace insurance, Medicaid, or hospital charity eligibility. Patients who qualify for charity might not always have a 100% of bill covered. Those who have an outstanding bill may be able to receive a payment plan on a case-by-case basis. For patients with insurance: program does not cover out of pocket expenses such as co-pays, co-insurance, and/or deductibles.

Eligibility

Well baby care and developmental assessments for ages through 3. Pediatrics and childhood immunizations for ages through 17. Other services have no age restrictions. Adolescent/adult immunizations from ages 13 up.
