Eligibility
  • Serves residents of Medina County on Medicaid.
  • Serves those on a Medicaid HMO who have reached their yearly limit of rides.
  • Also serves Medicaid recipients who are not on a Medicaid HMO and who are eligible for medical transportation assistance through Medicaid.
  • Transportation is for medical appointments only. Must be ambulatory.
Required documents
Will need proof that the client has reached their yearly ride limit through their Medicaid HMO (agency will advise).

Call the office for more information. Contact at least two or three (2 or 3) days in advance of appointment.
Fees
No fee to Medicaid recipients who are on a Medicaid HMO. Also serves Medicaid recipients who are not on a Medicaid HMO (sometimes called "fee for service") and who are eligible for medical transportation assistance through Medicaid.
