- Serves residents of Medina County on Medicaid. - Serves those on a Medicaid HMO who have reached their yearly limit of rides. - Also serves Medicaid recipients who are not on a Medicaid HMO and who are eligible for medical transportation assistance through Medicaid. - Transportation is for medical appointments only. Must be ambulatory.
Will need proof that the client has reached their yearly ride limit through their Medicaid HMO (agency will advise).
