Pre-paid call ahead same day service available. Walk-in/Same Day Service/Kiosk Service: Visit during regular Vital Records office hours of operation. Mail Order: Send a written request that includes the name, date of birth or date of death of the person on the certificate, daytime contact telephone number, and self-addressed stamped envelope to the office at 1867 West Market Street, Building A, Akron, Ohio 44313. Mail orders processed the same day they are received. Web/Internet Orders: Go to website at www.scph.org/vitals. Orders are processed the next business day. Phone Orders: Call the Office of Vital Statistics at 330-812-3845. Orders are processed for same day pick up or mail delivery.
