Proof of identity (photo ID, social security card, birth certificate). Proof of residency (utility bill). Proof of income (two pay stubs, social security or worker's comp income, unemployment).
Hours
Mon-Thu 8 am-11:30 am, 1 pm-4 pm. Fri 8 am-11:30 am, 1 pm-3 pm. Walk-ins accepted starting at 7:30 am.
Summer Dental Clinics (extractions only), registration begins at 4:30 pm on the following dates: July 15th and 29th, August 5th and 19th, 2024.
