Medicaid Health Insurance - Ages 19-64; Through 138% Fpl; Not On Medicare | Portage County Department Of Job And Family Services

Provides public insurance to cover medical services which may include doctor visits, some prescription drugs, hospital care, home care, long-term care, medical equipment, transportation, mental health, vision, dental, substance abuse services and other services. Most clients will have to choose and enroll in (will have their care provided by) a Managed Care Plan (MCP).

Application process

- Clients may call the Ohio Benefits Phone Line Contact Center Monday through Friday from 8:00am to 4:00pm to begin the application process, renew, and/or update their case information. May also apply, renew, and/or update case information through website anytime. NOTE: Client must have an email address to do this. - If preferred, may also pick up and drop off paper applications Monday through Friday from 8:00am to 4:30pm and submit documents on-site. - NOTE: MCP-eligible clients must select and enroll in an MCP, or will be auto-enrolled randomly (plan can be changed with reason, or during open enrollment).

Fee

- No fee to apply. - Once receiving coverage, individual MCPs may charge co-payments for some services.

Eligibility

- Serves residents of Portage County and be a United States citizen. - Serves adults age 19 to 64 who are not currently receiving Medicare. - Financial eligibility is based solely on income, and is set at 138% Federal Poverty Level (FPL).
