Required documents
Photo ID. Social security card for everyone in household. Proof of medical insurance.
Hours
Intensive Outpatient Program group Mon, Tue, Thur 9:30am-12:30pm or 5:30pm-8:30pm
Currently all groups are by way of telehealth and offered Monday -Saturday.

Fees
Services are free to Medicaid clients. Fee schedule available upon request.

Offer counseling services for chemical depency, assessments, treatment, mental health (grief loss, depression/anxiety, trauma/PTSD, addiction/co-dependency, sexual abuse and sexual addiction). Spiritual Counseling (upon request). Individual and couples counseling.
