- Must live in Tuscarawas County and be a United States citizen. - Serves adults age 65 or older and considered legally blind or disabled, or persons of any age who receive SSI or SSD. - Financial eligibility takes into account both income and resources and is set at 75% of the Federal Poverty Level (FPL). NOTE: Anyone who already receives any dollar amount of SSI will automatically qualify for Medicaid without filing a separate application or meeting an asset test.
Social Security Card, birth certificate, photo ID, Medicare Card, Proof of Income and Resources, verification of life insurance, if applicable.
