The application process may be started online through the Ohio Benefits self-service portal at https://Benefits.ohio.gov, over the phone through the Ohio Benefits Hotline, or in person. Arrangements will then be made with agency to complete application.

Eligibility
- Must live in Tuscarawas County and be a United States citizen. - Serves adults age 65 or older and considered legally blind or disabled, or persons of any age who receive SSI or SSD. - Financial eligibility takes into account both income and resources, and is set at 75% Federal Poverty Level (FPL). NOTE: Anyone who already receives any dollar amount of SSI will automatically qualify for Medicaid without filing a separate application or meeting an asset test.
