Serves ages 55 and older or ages 18 and over with a physical or mental health disability. Targets speakers of languages other than English, including Chinese (Mandarin and Cantonese), Cambodian (Khmer), French, German, Hungarian, Serbo-Croatioan, Tagalog (Phillipines) Ukranian, Korean, Karen (minority language of Burma/Thailand), Japanese, Spanish, Russian, Arabic, Hindi, Vietnamese, Bhutanese, and Nepali.
Will need the following information: Photo I.D., date of birth and Social Security number for self and spouse, State and ZIP code, Veteran status for self and spouse, length of time in current residence, a list of the prescription medications currently taking, including dosages, current income from all sources for self, spouse and others in the household, and an estimate of current bills such as mortgage/rent, utilities, medical bills etc. Will be advised if other documents/information are needed. Those needing tax preparation help should plan to bring relevant documents proving their income for the tax year they are filing for.
Benefits Screening is year-round by appointment M-F: 9:00am-5:30pm. Sa: 10:00am-4:30pm.

Description

Offers one-on-one assistance to help individuals determine which private, public, state and local benefits they may be eligible to receive. Includes benefits such as Medicaid, Medicare Part-D, SSI, SSD, SNAP (food stamps), HEAP, Golden Buckeye Card, Homestead Exemption, prescription assistance programs, housing and others. Will assist clients with filling out and submitting applications and forms and provides follow-up with client to see if they were able to enroll in the programs. Uses the Benefit Bank screening tool to provide screening and submit electronically applications for some government benefits such as Food Stamps, Medicaid and OWF.
