- Complete online application. - If the application is approved, a unique e-voucher number is emailed with a link to program's eyeglass ordering site and instructions on how to order eyeglasses.
Fee
$10 non-refundable application fee.
Eligibility
- Net monthly household income below 250% of U.S. Poverty Guidelines. - Eyeglass prescription from the last 24 months. New Eyes does not provide or pay for eye exams. - Must have pupillary distance measurement.
