Application required. Applications can be completed online, faxed, mailed, or delivered in person. Applications can take up to 30 days to process. Individuals who want to apply for Medicaid only can apply online at benefits.ohio.gov. Applications can be faxed from any Public Library of Cincinnati and Hamilton County branch for free. Mail: Applications can be mailed to 222 East Central Parkway, Cincinnati, OH 45202. Open to Hamilton County residents who are low-income adults and children, pregnant women, older adults and people with disabilities, and children who have aged out of foster care up to age 21. Households must have an income at or below 138% Federal Poverty Line.
