Eligibility
Lucas County Medicaid recipients. Serves those on a Medicaid HMO who have reached their yearly limit of rides (if they have not yet reached their yearly limit, they must contact their Medicaid HMO). Also serves Medicaid recipients who are not on a Medicaid HMO (sometimes called "fee for service") and who are eligible for medical transportation assistance through Medicaid. Transportation is for medical appointments only. Must be ambulatory (able to get into and out of a cab without assistance).
Required documents
Will need proof that the client has reached their yearly ride limit through their Medicaid HMO (agency will advise).

Fees
Free to Medicaid recipients who are on a Medicaid Managed Care Plan (MCP). Also serves Medicaid recipients who are not on a Medicaid MCP (sometimes called "fee for service") and who are eligible for medical transportation assistance through Medicaid. Transportation is for medical appointments only.
