Provides replacement funds to Food Assistance (food stamp) recipients who have lost food due to a power outage, fire or governor declared emergency. Replacement funds may replace up to (but not exceed) the client's monthly allotment of food assistance.

Application process
If the client is able, they can get the JFS 07222 form online. Applications are available for download through the Cuyahoga County Form Library (select "Food Assistance" in the drop-down menu under "Topic" in the search filter, hit the search button, and then scroll to "Replacement SNAP Benefits Request Form"). Paper application forms can be obtained M-F: 8:30am-4:30pm at any Neighborhood Family Service Center (NFSC). In most cases, client must return application to the agency (on site or by mail) within 10 calendar days of the reported loss. Once complete application is submitted, a prorated credit will be added to the Direction Card generally, within 24 hours after the start of the next business day (however, this may be longer when there is a high volume of applications). If needed to obtain verification of the outage, the client can phone their utility provider's customer service department to request verification of the outage and its duration (verification will then be mailed or faxed to the client). Clients may also apply over the phone by calling the Eligibility Contact Center: M-F: 8:00am-2:00pm.

Serves Ohio Direction Card (Food Stamp/SNAP) recipients residing in Cuyahoga County who have lost food due to a power outage, fire, or flood. In the case of a loss of electricity - power must have been out for at least four hours or more due to a situation beyond the client's control (a shut-off due to a lack of payment does not count). Will only help with replacement due to flooding or other disaster if the incident is a governor declared emergency. Replacement funds may be granted only twice in a six month period.
