The Patient Navigator program helps patients, families, and caregivers navigate the many systems needed during the cancer journey. A Patient Navigator will:
*Provide individualized assistance and support to patients, survivors, families, and caregivers;
*Facilitate access to American Cancer Society information and services, as well as provide access to additional community-based resources;
*Provide support and practical assistance in navigating required systems, such as financial, health care, and social systems;
*Provide information and education about the cancer journey;
*Assist in identifying strategies that help to ensure a better quality of life.
