- Serves residents of Portage County age 60 or over at the time of the application with an annual income at or below 185% of the Federal Poverty Guidelines.

Application process
- Call first Monday through Friday from 8:00am to 3:00pm to confirm that the applications are available. After client confirms that the applications are available, they may walk in to pick up the application and receive information on how they can receive the vouchers.
- Anticipate delivery sometime between June 21st to July 2nd.
- Those that live in a senior residential/apartment community, envelopes may be delivered by their manager.
- Applicants may get a call from a volunteer to schedule a delivery. The volunteer may ask you to sign upon receipt.
- Please wait until after the designated delivery schedule before calling the office. The agency will not have information regarding delivery until everything is final. If, after the 4th of July holiday an applicant still has not received their coupons, arrangements can be made to pick up the coupons at the agency's office.

Provides older adults $50 to be used across the state for fresh locally-grown produce and herbs at area farmers markets and roadside stands. Participants have the choice to use a mobile application or a physical card.
