Safe Haven I/ Safe Haven Iii | Frontline Service

Provides permanent housing in a supervised environment with supportive services to adults who have a major mental illness and are homeless. Offers case management, assistance with activities of daily living, and one meal a day. Residents are supervised from morning to evening.

Application process

Case workers may phone or fax referral M-F: 9:00am-5:30pm; Those who are making a self-referral may also phone M-F: 9:00am-5:00pm to be linked with a doctor and case worker who will make the referral. Written referral needed with doctor's signature and diagnosis.

Fee

Fees are 30% of income for rent plus nominal charges for utilities and phone; most clients receive SSI for mental disabilities, case worker will help those who do not receive SSI apply.

Eligibility

Males and females 18 and older in Cuyahoga County who have a major mental illness such as schizophrenia, bipolar disorder, or severe depression and are homeless. Must meet the HUD definition of homelessness.
