Description

Providing ongoing in-home needs assessment from licensed social workers, care planning, and consultation to help vulnerable seniors maintain their independence through Senior Supportive Services. Assistance is provided for financial management, medical appointments, insurance, transportation, and housing needs, and by linkage to other community services.
